You can print these charts or upload them directly to Mangomint's Integrated Forms feature so your team and clients can mark them up digitally. Forms & Charting is included for all Mangomint customers, and setup takes four steps:

  1. Create a form template and add the Image Markup field.

  2. Choose Template to upload one of these charts, or choose User provided to use a client’s photo.

  3. Select the markup tools you want to allow.

  4. Turn on Require this field if the image must be added and marked up.

You can add several Image Markup fields to one form if you need more than one chart. The Image Markup help article covers each setting in detail.

Face charts for med spas and injectables

Injectables are where face charts earn their keep in medical spas. The American Society of Plastic Surgeons (ASPS) reports neuromodulators and hyaluronic fillers made up 80% of minimally invasive procedures in 2025 (read: lots of charting).

Digital charting can help your med spa team in a few ways:

  • Clients can mark problem areas or sources of pain on a chart, which helps you map treatment areas and tailor the service.

  • Injectors can circle each injection site and log units in a comment. The FDA says Botox should come from a licensed, trained provider.

  • Clients can upload their own photos with an intake form, and you can track before-and-after photos over time.

Our video, Image Markup for Med Spas, shows how to identify injection sites and track client progress.

Face charts for tattoo and piercing studios

In tattoo and piercing studios, a face chart gives you a clear record of placement. For the rest of your paperwork, see our tattoo intake form examples. Here’s where one fits:

  • Clients can add their own photo and mark where they want a new piercing before the appointment.

  • Your techs can mark up an uploaded chart to plan and document permanent makeup services.

  • You can request a chart form automatically when someone books a facial tattoo, to collect preferred size and placement.

Salon face charting for beauty and waxing

Beauty salons can put these templates to work too, and that includes lash and brow businesses. They’re also handy for waxing & sugaring appointments. Each can use them a little differently:

  • Makeup artists can use a chart to plan new looks, and clients can mark one up to show the look they want.

  • Waxing clients can mark specific areas on a chart in your intake form, adding detail that a treatment-area booking leaves out.

That detail can help you tailor services for each client, giving them exactly the results they're looking for.

Face and body charts for day spas and massage

Day spas share many face charting needs with salons and medical spas, with additional considerations for massage services. Some ideas to start with:

  • Clients can flag problem areas before a facial massage, so you can prep the service and make the most of the appointment time.

  • A facial muscle chart can show clients which areas you’ll work on, such as during a gua sha facial.

  • Massage therapy professionals can also use full-body charts to map the areas clients want addressed.

For massage-specific ideas, watch Image Markup for Massage Therapy Practices. It shows how to identify focus areas and pain points, then track progress.

    What is a face chart?

A face chart is an outline of a face that you draw on to plan a service or record one after it’s done. Beauty and wellness businesses use them to capture details that are hard to describe in words.

How is a makeup face chart different from a Botox chart?

A makeup chart helps an artist plan a look or show a client options. A Botox chart is a clinical record of each injection site and the units used there.

How do you fill out a Botox face chart?

Mark each injection site on the chart and note the units used at that site. For anything else you need to record, follow your medical director’s protocols and your state’s requirements.

Can you use face charts digitally in Mangomint?

Yes. Upload a chart as the template in an Image Markup field, and your team or clients can mark it up on desktop or mobile. Staff can also add comments, such as units for injectable services.

Can clients fill out a face chart before their appointment?

Yes. Clients can add their own photo to a client form and mark it up before they arrive, without logging in to an account. You can also send forms automatically when specific services are booked.

Where are completed face charts stored?

Submitted forms, including marked-up charts, sync to the client’s profile in Mangomint. HIPAA compliance isn’t turned on by default, so request HIPAA enablement and sign a business associate agreement (BAA) if your business needs it.
