A body chart template is a blank body outline that med spa providers and massage therapy practices mark up to document treatment areas and client concerns. These 8 free templates also suit day spa teams, and you can use each one on paper or with Mangomint's Image Markup feature.

Before-and-after body chart template

A before-and-after body chart puts two gender-neutral figures side by side, each with fields for circumference measurements and weight. Record the numbers at the first visit and again after the treatment series, then mark either figure to note other changes, such as hair coverage or skin texture.

Musculoskeletal body chart template

Whether you are charting out massage muscle targets or injection sites, a musculoskeletal full-body chart can help. These templates let you add visual indicators alongside your service area notes.

Gender-neutral front and back full-body chart

A front and back body chart, also called a body map, helps you and your client point out treatment areas during intake or consultations. This gender-neutral version fits most clients and services, and its dashed guide lines help you place each note precisely.

Masculine and feminine full-body chart

For some services, a gender-neutral outline can miss body contours that matter to the treatment area. This chart includes a masculine and a feminine form, so clients can choose which one to mark up during intake.

All-sides body chart

This chart adds left and right profile views to the standard front and back, which helps when a client's pain wraps around the body. Side views also help clients who aren't sure whether the back view is mirrored (no more guessing which side is left). Massage therapists can use the profile views to document posture work, too.

Body measurement charts

Body measurement charts help med spas track changes over a treatment series for body contouring services like CoolSculpting. Measurements are also useful for mapping laser hair removal areas or recording the size and placement of a tattoo.

Male and female measurement chart

This chart places a female-presenting and a male-presenting figure on one page, each with labeled circumference lines. If you serve a mix of clients, one template helps keep your measurement records consistent.

Female measurements body chart template

If your business mainly serves women, a female-specific measurement chart gives clients a figure that better reflects their body. This version shows one arm raised and one lowered, with guide lines for each measurement point.

Male measurements body chart template

A male-specific measurement chart helps you tailor treatment plans for male clients. It's a practical choice for med spas and waxing studios with a large male clientele.

How to use full-body charts

To chart digitally, add an Image Markup field to a form template in Mangomint Forms and Charting. Then set any of these body charts or our face chart templates as the field's image. You can choose which markup tools appear on each form, such as freehand drawing or arrows.

Clients can mark up a template on their intake form, like circling a sore shoulder before a massage. On internal forms, your team can add comments with details such as the units used for an injectable.

Image Markup works on client photos, too. Marked-up images save to the client profile, and staff charting in Mangomint is HIPAA compliant. Integrated Forms is free for all Mangomint customers.

For a walkthrough, watch the Image Markup video for med spas or read our step-by-step Image Markup guide. If you document medical treatments, have your medical director or another qualified reviewer approve how your team uses body charts in client records.

Frequently Asked Questions

What is a body chart used for?

A body chart gives you and your client a shared picture of where treatment happens. Med spas use body charts to map treatment areas, and massage therapists use them to record where a client feels pain or tension.

What's the difference between a body chart and a body measurement chart?

A body chart is an outline you mark up to show locations, like a treatment area or a sore spot. A body measurement chart adds labeled lines or fields for circumference measurements, so you can compare numbers across a treatment series.

Can clients fill out a body chart before their appointment?

Yes. In Mangomint, you can add an Image Markup field to a client intake form so clients can mark up the chart before they arrive. The completed form saves to the client profile for your team.

Which body chart works best for documenting pain?

An all-sides body chart gives clients four views to work with, including both profiles. For a quick intake sketch, a front and back chart covers most needs.

How should I store completed body charts?

If your business handles protected health information, keep completed charts in a secure place and limit who can view them. Mangomint saves marked-up charts to the client profile, and its staff charting is HIPAA compliant. For paper charts, review your storage process with a compliance advisor.
